Provides Supported Housing functions as outlined by the Texas Administration Code following Resiliency and Disease Management Principles to include treatment planning, monitoring of services, and the provision of Supported Housing services. Develops and maintains referral resources for service provision allowing for consumer choice, where available of providers. The Supported Housing Coordinator ensures the best value of resource allocation to service provision within a recovery-based framework. Will provide Tenant-Based Rental Assistance to disabled adults in a community setting when appropriate based on eligibility standards. Will work under moderate supervision with latitude for the use of initiative and independent judgment when assisting an individual in obtaining secure and affordable housing. Will implement the principles of trauma-informed care including cultural sensitivity in all areas of service delivery.
This position will also manage the MHMR owned Windy Meadows Apartments and will submit reports to the managing company, Fairmount Management Company. These duties include, but are not limited to, being knowledgeable about HUD rules, HUD billing, collection and depositing rent payments, and utilizing ICAM program to document the recertification of residents at Windy Meadows.
EDUCATION REQUIRED: Should have college experience from an accredited college or university and hours should evidence a pursuit of education in the field of psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology. High School diploma required.
LICENSE/CERTIFICATION REQUIRED: Valid Texas driver’s license and must be insurable on the Center’s insurance. No more than 3 moving violations in the last 3 years, must have proof of current liability insurance. Complete training in accordance with TAC 412.316. Documented training with SAMHSA Permanent Supported Housing prior to the provision of Supported Housing services. Obtain Certification as an Assisted Housing Manager after hire.
EXPERIENCE REQUIRED: At least three years of documented service in case management and rehabilitative services with MHMR Services for the Concho Valley preferred.
Certification in CPR/AED, First Aid, and Prevention and Management of Aggressive Behavior (PMAB). Other training and/or certification deemed necessary by TXMHMR or this Center.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED: Must be capable of using Microsoft Office products including Excel, Outlook, and Word. Strong organizational skills. A demonstrated ability to work in a fast-paced environment. Ability to clearly communicate sensitive information both orally and in writing. Demonstrate the ability to work closely with all levels of service providers in an atmosphere of teamwork. Ability to manage multiple tasks simultaneously.
