|Title:||CFC Service Coordinator|

Provides service coordination functions as outlined by Texas Administrative Code, Title 42, Chapter IV, Subchapter C, Part 41, Subpart K which includes assessment for the need of personal assistant services, emergency response services, day habilitation services, or support management services; treatment planning which will include person centered thinking, and monitoring of services. Must maintain an outgoing working rapport with Managed Care Organizations (MCO's), Providers, Individuals, and Legal Authorized Representatives. Develops and maintains referral resources for service provision allowing for consumer choice, where available, or providers. Authorizes provision of services based upon assessed need and availability and ensures best value of resource allocation to service provision.
EDUCATION REQUIRED: Bachelor or advance Degree from an accredited college or university with a major in social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education and criminal justice or be a licensed chemical dependency counselor physician assistant.
LICENSE/CERTIFICATION: Valid Texas driver's license and must be insurable on the Center's insurance. No more than three moving violations in the last three years.
EXPERIENCE AND SKILLS REQUIRED: The individual must demonstrate, either through experience or education, the ability to work in a rapid paced environment in an organized and professional manner; have the ability to self-monitor production and quality with minimal assistance; have basic knowledge and familiarity with data systems including business software such as Microsoft; willingness and ability to learn the CARE system and Cerner; and have the ability to work as a team member in a diverse environment interfacing with both internal and external customers.
