The employee provides Case Management functions as outlined by Texas Administrative Code following Resiliency and Disease Management Principles. This includes assessment, treatment planning, monitoring of services, crisis services and the provision of Skills Training as assigned and specified. Caseworkers should develop and maintains referral resources for service provision allowing for consumer choice, where available, of providers. This employee works to ensures best value of resource allocation and quality of services provision within a recovery-based framework. Caseworkers observe individuals in a variety of situations for background information, monitoring of progress and treatment services. Assist individuals and families to access medical, transportation, social, educational and other appropriate services that will help the child achieve a quality of life and level of care acceptable to the family. Provide Crisis prevention and management services to include location and coordinating services and supports to prevent or manage a crisis. The employee will provide wraparound facilitation and intensive casework to individuals within the MHMRCV catchment area who are identified as at risk for placement outside of the home.
ADDENDUM: As a Caseworker II, this employee may be as point of contact should the Intensive Services Supervisor and the MH Program Manager be out of the office or otherwise unavailable.
EDUCATION REQUIRED: Bachelor’s Degree in a Center approved Human Services field. Bachelor or advanced Degree from an accredited college or university with a major in a social, behavioral, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and education.
LICENSE/CERTIFICATION: Valid Texas driver’s license and must be insurable and remain insurable on MHMR Services for the Concho Valley center’s insurance. Must not have more than 3 moving violation in the last 3 years, and proof of current liability insurance. Must be willing to work towards certification as a Qualified Mental Health Provider (QMHP-CS) Will work towards NWIC certification as a wraparound facilitator to accomplish additional job duties.
EXPERIENCE REQUIRED: Preference will be given to previous experience working with children/adolescents, families, schools and the Juvenile Justice System. A minimum of six months or more working with or in the field of mental health/illness or social work experience is preferred.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: SPECIAL SKILLS: A demonstrated ability to work in a fast-paced environment as a team member. Willing to work in an accountable system where goals for performance are established, measured and being held accountable for meeting or exceeding those established goals. Ability to self-monitor production and quality with minimal assistance. Basic knowledge and familiarity with data systems including business software such as Microsoft Word. The ability to work as a team member in a diverse environment interfacing with both internal and external customers. Understand and implement the wraparound process according to Wraparound Fidelity.
