The employee provides case management functions as outlined by Texas Administrative Code following Resiliency and Disease Management Principles. This includes assessment, treatment planning, monitoring of services, crisis services and the provision of skills training as assigned and specified. Case managers should develop and maintain referral resources for servicees provision allowing for consumers choice, where available, of providers. This employee works to ensures best value of resources allocation and quality of services provision within a recovery-based framework. Case managers observe individuals in a variety of situations for background information, monitoring of progress and treatment services. Assist individuals and families to access medical, social, educational and other appropriate services that will help the child achieve a quality of life and level of care acceptable to the family. Provide Crisis prevention and management services to include location and coordinating services and supports to prevent or manage a crisis.
ADDENDUM: The Certified Family Partner is an active member of all MHMRCV Child Family Teams (LOC 4 and LOC YES) unless deemed unnecessary by the CFT itself. This employee teaches self-advocacy skills to families, assist in engaging families in the treatment process, serves as a liaison for family, staff, schools, and communities, provides mentoring, informal supports, and referral for individuals they are assigned. Identifies natural/traditional supports for families and provides non-clinical skills training for families.
EDUCATION REQUIRED: Bachelor's degree in a Center approved Human Services field. Bachelor or advance Degree from an accredited college or university with a major in social, behavorial, or human service field including psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, and education.
LICENSE/CERTIFICATION REQUIRED: Valid Texas driver’s license and must be insurable and remain insurable on MHMR Services for the Concho Valley center’s insurance. Must not have more than 3 moving violations in the last 3 years, and proof of current liability insurance. Must be willing to work towards certification as a Qualified Mental Health Provider - CS.
ADDENDUM: Will work towards and maintain via Hope cerification as a Certified Family Partner within one year.
EXPERIENCE REQUIRED: Preference will be given to previous experience working with children/adolescents, families, schools and the Juvenile Justice System. A minimum of six months or more working with or in the field of mental health/illness or social work experience is preferred.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: A demonstrated ability to work in a fast-paced environment as a team member. Willing to work in an accountable system where goals for performance are established, measured and being held accountable for meeting or exceeding those established goals. Ability to self-monitor production and quality with minimal assistance. Basic knowledge and familiarity with data systems including business software such as Microsoft Word. The ability to work as a team member in a diverse environment interfacing with both internal and external customers.
