The employee will provide recovery-oriented services as outlined by the Texas Administrative Code and in accordance with the Texas Health and Human Service Commission Resiliency and Recovery Management principles. MH recovery services assist an individual in gaining and coordinating access to necessary care and services appropriate to the individual’s needs. Recovery Management functions include: patient assessment, treatment planning and monitoring of services; crisis services, provision of skills training. The Case Manager will develop and maintain referral resources for service provision allowing for patient choice. Service focus is on the reduction of intensity and frequency of symptoms, increase in self-reliance, and improvement of independent living skills and community interactions. The employee will implement the principles of trauma-informed care including culture sensitivity in all areas of service delivery.
EDUCATION REQUIRED: A bachelor’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology.
LICENSE/CERTIFICATION: Valid Texas driver’s license and must be insurable and remain insurable on MHMR Services for the Concho Valley center’s insurance. Must not have more 3 moving violation in the last 3 years, and proof of current liability insurance. Must be willing to work towards certification as a Qualified Mental Health Provider-CS.
EXPERIENCE REQUIRED: One-year minimum experience in like or similar field preferred.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: SPECIAL SKILLS: Must be capable of using Microsoft Office products including: Excel, Outlook, and Word. Strong organizational skills. Ability to self-monitor production and quality. A demonstrated ability to work in a fast-paced environment. Ability to clearly communicate sensitive information both orally and in writing. Demonstrate the ability to work closely with all levels of service providers in an atmosphere of teamwork. Ability to manage multiple tasks simultaneously.
