The Service Coordinator, with information provided via the initial intake and screening, develops a treatment plan, coordinates the services received, monitor the services received and makes recommendations for changes in services.
Continuity of Services Services are provided primarily for individuals being transferred from one service to another (such as from inpatient to outpatient). A designated Qualified Mental Health Professional ensures, via concurrent planning and frequent contact, that services are consistent and follow the treatment plan from one service/program to the next.
Psychiatric Services Qualified Licensed Physicians provide needed services for initial and ongoing treatment via Psychiatric Evaluations and medication prescriptions.

The primary target population for these services is historically high resource utilizers who without these services would be institutionalized.
Supported Housing Supported Housing services are designated to assist an individual with a severe and persistent mental illness in securing and maintaining housing within the community.
Supported Employment Supported Employment services provide job related services such as job coaching, job skills training, interview skills training and job search activities for individuals receiving services from the Center.
Community Intervention Community Intervention provides individuals with rehabilitative services within the community. The services are not site based services but occur in the natural environment of the individual. The services may include life skills training and symptom management.
